Shared Living Provider (Independent Contractor)

Qualifications: 
  • High School diploma or equivalent
  • Valid Drivers License & an insured vehicle
  • Experience advocating for and supporting persons with developmental disabilities is preferred

Support: 

The provider receives supports and services from the Shared Living Program Coordinator.

 

General Roles & Responsibilities

 

1.      Develop a relationship with an individual with a developmental disability, which is based on mutual respect and dignity.

2.      Provide or create a safe and comfortable home with the individual, which accommodates any specific needs of each person sharing the home. 

3.      Foster community involvement and relationship building by providing opportunities within the providers’ existing social network

4.      Encourage and foster the individuals’ existing significant relationships with families and friends.

5.      Participate in the implementation of the Individual Service Plan (ISP).  Assist in achieving personal goals through teaching and support including monitoring of behavior/clinical plans.

6.      Perform related record keeping and documentation.

7.      Provide necessary transportation.

8.      Attend and participate in regular provider meetings and consultation.

9.      Participate in mandatory and needed trainings including First Aid/CPR, Fire Safety, and Human Rights.

10.  Communicate regularly with the Shared Living Coordinator.

11.  Ensure a safe environment, including providing crisis intervention and assessing emergency services as needed.

12.  Provide other related supports to the individual as needs arise.

 

Compensation 

Stipends are negotiated based on the individual needs of the person.
