Senior Support Services

Senior Support Services at Advocates Community Counseling is dedicated to serving older adults and their caregivers so they may lead rich, fulfilling, and productive lives.

Clinical Services:

  • Assessment and evaluation
  • Individual, family, and group counseling in a safe, caring environment
  • Medication evaluation and management
  • Crisis Intervention
  • Expressive Arts Program
  • Conflict resolution
  • 24-hour crisis hotline

Support Services:

  • Nursing home diversion through clinical stabilization
  • Adult Foster Care services (AFC certified)
  • Elder Community Care in collaboration with Baypath Elder Services
  • Shared living support
  • Referrals for legal, medical, financial, physical, social or mental health needs
  • Coordination of care with other service providers in the elder's life

The Elder Community Care Program:

Funded by a 3-year, $1.2 million federal grant from the Substance Abuse and Mental Health Services Administration, Advocates new Elder Community Care Program expands prevention, screening and treatment services for older adults dealing with issues such as depression, anxiety and substance abuse. Advocates works in collaboration with Bay Path Elder Services, Samaritans, Boston College, and Advocates for Human Potential to provide evidence-based treatment to elders who are not currently connected to behavioral health assessment and treatment services. Services are tailored for seniors who are at risk for, or are experiencing, mental health and/or substance abuse problems.

3D Depression Screening Intervention for Suicide Prevention:

Designed in partnership by Advocates, Inc. and Baypath Elder Services, this Department of Public Health-funded program includes three depression screening strategies - training, screening and intervention - to prevent elder suicide. Specially trained Baypath case managers and outreach worker screen elders living in the community for depression. Those with positive scores are offered in-home joint-intervention by an Advocates' Senior Support Services clinician and their case manager/outreach worker. During the intervention phase, the clinician assesses safety, evaluates risk factors, and provides additional information on treatment options to elders and their caregivers. Appropriate community resources and follow-up are initiated, including an evaluation with our Psychiatric Emergency Services team if clinically appropriate.

Advocates Staff

Advocates clinical staff provides compassionate, professional, supportive services to seniors and their families to facilitate maximum independence and to help clients reach and maintain their fullest potential with dignity. Anyone can make a referral for service including the elder him or herself, community providers, family members, hospitals, or Councils on Aging.

All services are voluntary and confidential; we always seek the full participation of the individual in his or her treatment process. Therapeutic services are billed to seniors' insurance or private payment arrangements can be made.

The team is able to see elders in the office, at their homes or at a day program in order to facilitate a comfortable and therapeutic environment. In addition to English, we provide services in Russian and Spanish. The SSS service area includes, but is not limited to, 14 MetroWest communities: Ashland, Dover, Framingham, Holliston, Hopkinton, Hudson, Marlborough, Natick, Northborough, Sherborn, Southborough, Sudbury, Wayland, and Westborough.To inquire about services or make an appointment, contact:

  • Youth in transition from foster care
  • Individuals and families who are homeless or at risk for homelessness
